Absolutely.
Actually it is possible to create a custom distribution out of every available flavor of Linux.
Possible, yes. Time consuming? Potentially, very.
The more you deviate from base, the more time you need to spend.

...how do I do it?

The hard way. You start from the base and build things up to your liking.
Creating the actual .img file is just the smallest and less time-consuming step.
Careful not to fall in any of the two black pits.
The first is marked "Purgatory of hardware incompatibility", the second "Infernal dependency hell".
Starting from Lite is a good way to avoid the first pit.
