Having got round to trying this, it looks as though HCI commands are failing, and that is why the kernel hasn't enabled hci0. This can happen if the power supply isn't good enough, or if there are too many devices plugged into the Pi. Note that although the Pi can detect and report some types of undervoltage, the lack of a warning does not mean that all is good.

If you have a spare SD card, install a clean RPiOS image and see if it can see any Bluetooth devices. Otherwise, this is looking like a hardware problem.
I've seen this issue in my raspberry pi 4B, I have a BK06 device connected, it is working fine in other 5 raspberry.
I have a single raspberry that has issues with this. In some boots, the device works flawlessly (no tx timeout in sight), in some reboots the device disconnects the bluetooth or in some cases a fresh boot never connects the bluetooth.

In dmesg I start seeing

 hci0: command 0xXXXX tx timeout
in the disconnection cases (and bluetooth reports being powered down, and can't get it back to power on).

I'm using this kind of power source, leveling it to 5.1 volts.
I'm now bringing it to 5.3v to see if this fixes the issue.

My setup also has this sim7600X waveshare hat attached

Can this be a power issue?
