All the USB ports stop functioning suddenly after some time. RPi doesn’t respond to any mouse or keyboard inputs or any other USB device. Any USB device connected after the error has occurred is not acknowledged.
Known old bug the Rpi USB system usual crashes at all. On other system a "faulty" device would just powercycle and work again, but not rpi.
That can be from noisy environment, voltage drops, bad cables, oxidation of the Plug of the device&Raspberry ports.
Cleaning ports&plugs, carefully bend the holders. Solder cables, sometimes Shild is just clamped in the Plug not solderd.
Sometimes the USB System crash too Ethernet, dont know why. Then a manual reboot isnt possible then cause you have
no access to the RPI. But you can write your own watchdog that do a reboot - if the system is not crashed badly and there is some memory left.

That reboot fail "kernel panic error" is new, why you reinstall the same system again and again? Use a older OS Version that dont have this new Bug.
