I believe I solved the issue, everything is/was configured properly.

During the boot sequence on the RPi the Pi drops the ethernet connection for about 5 seconds, when this happens there is a delay on the switch (maybe preservation mode?) that causes the switch not to recognize the RPi for a few seconds after it comes back up. This is apparently enough time to cause the RPi to fail the boot sequence.

On the (cisco) switch that I'm using I told the switch to ignore any carrier transitions of less than 15 seconds (interface command - "carrier-delay 15"). After I put the carrier delay on the interface I've been able to successfully reload the RPi multiple times
I lied, this didn't fix it
