It looks like you're missing a label for U4 connecting Pin 20 (PRTPWR4/BC_EN4) to USB4_VBUS_EN, as has been done with the PRTPWR1-3 pins. Presumably, this means the power switch IC for port 4 is never enabled, so that port won't be able to provide power?
