Interesting comments.

I am not changing file systems as that would only hide the problem. I want to find the cause.

SMR is way to slow. One process that takes 40 minutes with SSD and 4 hours with CMR, ran for 5 days with SMR. I am phasing out SMR. Before tossing the disk in the bin, I want to find out why the disk fails in the Pi 5 but not anywhere else. My next tests will be with a different Pi 5 and with a Pi 4 for comparison.

The disk is WD. It is packaged in an enclosure so cannot be swapped to a different enclosure. I would not buy an enclosure for an SMR disk as SMR is junk.

My hub does not fail anywhere else and worked with a Pi 4 to host multiple disks. They were CMR. 1 TB disks in USB 3 enclosures. I used three to create 3 TB of storage.

I could move my process to a Pi 4 with the powered USB hub as a Pi 4 is faster than SMR. I could probably use my Pi Zero 2 as it would be faster than SMR, except for the lack of RAM. I need more than 0.5 GB of RAM.
I am using the 4-bay enclosure mentioned in

viewtopic.php?p=2103794#p2103794

with a BTRFS filesystem spread across four 8TB drives in mirror mode. It's been reliable with a Pi 4 for a number of years. That's why I mention it.

In my opinion using the 64-bit operating system is important for a NAS.
